The City of La Mirada requires the completion of a Wavier of Liability, Assumption of risk and Indemnity form prior to participation in classes, programs, and excursions.

All participants under the age of 18 must have both legal guardians sign a wavier prior to participation in classes and programs. Each time a participant registers for a class or program, a new waiver must be filled out.

Waivers can be obtained from the Resource Center, Activity Center, Gymnasium and city website.

Waivers can be hand delivered or mailed to the Resource Center, 13710 La Mirada Blvd. Faxed copies will not be accepted.
